Facial Mole Excision (Male, 50s)|Punch Excision with Natsui-Style Moist Wound Healing — 8-Year Scar-Free Result [Osaka, Shin-Osaka]

Preoperative photo of facial mole excision. Male in his 50s with a mole present for over 10 years that recently enlarged.

Hello, this is Dr. Hirotaka Kohrita,
director of Kohrita Orthopedic and Plastic Surgery Clinic,
located in Shin-Osaka, Yodogawa-ku, Osaka.

In 2017, a male patient in his 50s underwent
facial mole excision at our clinic.

Eight years later, he returned for the treatment
of a neck epidermoid cyst.

Upon examination, we found that even after eight years,
the surgical scar was almost completely invisible.

In this article, I would like to present the long-term healing outcome
of this mole excision case.

Case Overview and Surgical Method (2017)

Eight years ago,
the patient visited our clinic with a mole on his face
that had been present for more than 10 years
and had gradually enlarged.

The procedure was performed under local anesthesia using
the punch excision technique (known in Japan as “Kurinuki-hō”).

This minimally invasive method allows the mole to be removed
through a small circular incision, without making a large linear cut
— thereby minimizing trauma and postoperative scarring.

After the excision, the wound was managed using
the Natsui-Style Moist Wound Healing Method,

which involves covering the area with a hydrocolloid dressing
to maintain a moist environment and promote natural healing.

No disinfection was performed, and the patient was allowed to
shower and bathe daily from the day after surgery.

Antibiotics were not prescribed,
and there was almost no postoperative pain.

The postoperative course was smooth.
The 2017 blog article documented the entire healing process
with photographs from preoperative to 1 month after surgery.

Eight-Year Follow-Up (2025)

Eight years after facial mole removal using punch excision. No visible scar, excellent cosmetic result.

8 years after punch excision: The scar is barely visible, natural appearance.

Eight years later, when the patient revisited our clinic
for the neck cyst surgery,

the site of mole excision was examined again.
The skin color and texture were extremely natural,
and even upon close inspection,

it was nearly impossible to tell where the mole had been removed.

The patient was very pleased, saying:

“I can’t even see the spot anymore.”

Such long-term, aesthetically excellent outcomes
offer great reassurance to patients.

Why Did No Scar Remain?

1. Minimally Invasive Punch Excision

The punch excision removes only the lesion itself in a circular fashion,
without large linear incisions.
This minimizes skin tension and leads to a smaller, less visible scar.

2. Natsui-Style Moist Wound Management

Instead of disinfection, the wound is kept moist using hydrocolloid dressings.
This method maximizes the skin’s natural healing ability,
allowing painless and beautiful healing.
Daily showers and baths are allowed immediately after surgery.

3. Appropriate Management Without Antibiotics

If no signs of infection are present, antibiotics are not prescribed.
We avoid unnecessary medication
and focus on the body’s innate healing capacity.

Frequently Asked Questions (FAQ)

Q. Will a scar always remain after mole removal?
A. It depends on the surgical method and postoperative care.
By combining the punch excision technique with
Natsui-style moist wound healing,

we achieve long-term results where scars are minimal or almost invisible.

Q. When can I shower or bathe after the procedure?
A. At our clinic, showering and bathing are permitted
immediately after surgery.

Keeping the wound clean with running water helps natural healing.

Q. Are antibiotics necessary?
A. Not unless there are signs of infection.
Most patients heal well without antibiotics.

Comparison: Before Surgery and 8 Years After

Preoperative photo of facial mole excision. Male in his 50s with a mole present for over 10 years that recently enlarged.

Before surgery: Enlarged facial mole (Male, 50s).

Eight years after facial mole removal using punch excision. No visible scar, excellent cosmetic result.

8 years after punch excision: The scar is barely visible, natural appearance.

Summary

In this case, even eight years after mole excision,
the surgical site remained virtually scar-free.
This excellent long-term result was achieved through
the combination of minimally invasive punch excision
and Natsui-style moist wound healing.

Such outcomes provide long-lasting reassurance and confidence for patients.
